Summary

1. This letter advises Primary Care Trusts and Island Health Boards of the introduction of a new form GOS(S)6A and a revised form GOS(S)2. It also provides information on the completion of GOS(S)6 and clarification on the completion of the revised GOS(S)3 and GOS(S)4.

2. Bob Anderson, Assistant Director (Dental and Ophthalmics), Practitioner Services Division wrote to Primary Care Trusts/Island Health Boards on 6 October advising that PSD will, with effect from 1 December 2000, issue list numbers for opticians and ophthalmic medical practitioners (OMPs) wishing to join Primary Care Trust/Island Health Board ophthalmic lists. A new form GOS(S)6A has been produced for this purpose. The GOS(S)6A will comprise sets of 3 NCR sheets with 50 sets to a pad.

3. A GOS(S)6A should be completed and sent to PSD by the Primary Care Trust/Island Health Board for every optician/OMP who wishes to join their list on or after 1 December 2000. All 3 sheets should be sent. PSD will then insert the list number onto the GOS(S)6A and retain 1 copy for their own records, return 1 copy to the Primary Care Trust/Island Health Board and send the third to the Scottish Executive.

4. Supplies of the GOS(S)6A will not be available until the week beginning 4 December. Initial supplies will be sent to Primary Care Trusts/Island Health Boards without requisition and further supplies can be obtained from The Stationery Office in the normal way. Until stocks are received Primary Care Trusts/Island Health Boards should continue to request list numbers from Lothian Practitioner Services Division in the same way as they are now.

GOS(S)6

5. To ensure that the information which opticians/OMPs are asked to provide when applying to join an ophthalmic list is consistent Primary Care Trusts/Island Health Boards are asked, with effect from 1 December 2000, to only issue form GOS(S)6 (Application for inclusion in the ophthalmic list of a Primary Care Trust/Island Health Board) to those wishing to join their lists. From that date any locally agreed applications should cease to be used.

6. To ensure that Primary Care Trusts/Island Health Boards have stocks of the GOS(S)6 initial supplies will be sent without requisition. Further supplies can be obtained from The Stationery Office in the normal way.

7. It was agreed that, once the revised GOS(S) forms 1, 3, 4 and 5 had been finalised, the GOS(S)2 should be amended to incorporate the new prescription layout. The GOS(S)2 has now been amended and stocks will be issued to Primary Care Trusts/Health Boards for onward transmission to ophthalmic contractors in due course.

8. Primary Care Trusts/Island Health Boards are asked to note the further information on the completion of forms GOS(S)3 and 4 contained in the Memorandum to this letter.

9. Copies of the Memorandum to this letter are being sent under separate cover for urgent distribution to all optometrists and ophthalmic medical practitioners on Primary care Trust/Island Health Board lists.

2. CLARIFCATION ON THE COMPLETION OF REVISED GOS(S)3 AND GOS(S)4

1. This Memorandum advises of the introduction of a revised GOS(S)2 and provides clarification on the completion of the revised GOS(S)3 and GOS(S)4.

GOS(S)2 - Patient's Optical Prescription or Statement

2. It was agreed that once the revised GOS(S) forms 1, 3, 4 and 5 had been finalised the GOS(S)2 should also be amended. The GOS(S)2 has now been amended and stocks will be issued to Primary Care Trusts/Health Boards for onward transmission to ophthalmic contractors in due course.

3. The only changes to the GOS(S)2 relate to the voucher and supplement field and the prescription, which now mirror those on the revised GOS(S)3 and 4. Instructions on how to complete the revised prescription were contained in the Annex to the Memorandum to NHS: 2000 PCA(O)9, issued on 16 October, and the Amendment to the Amendment to the Memorandum to NHS: 2000 PCA(O)9, issued on 10 November.

GOS(S)3 AND GOS(S)4

4. The Annex to the Memorandum to NHS: 2000 PCA(O)9, issued on 16 October, provided guidance on how to complete the revised GOS(S) forms. Paragraph 8 of the section headed "GOS(S)3 - NHS Optical Voucher and Patient's Statement" advised that Part 3 of the GOS(S)3 (and also Part 5 of the GOS(S)4) includes a new field "GOC Number". Practitioners were advised that this should be completed where glasses are supplied to children under 16 and the registered blind and partially sighted or where contact lenses are supplied. Practitioners should note that the GOC Number only requires to be inserted when the supplier does not have an ophthalmic list number. This is to ensure that unregistered suppliers are not dispensing glasses to children, the registered blind and partially sighted and contact lens users. Suppliers who are also sight testers, and therefore have an ophthalmic list number, do not require to insert their GOC Number.
