ACUTE SERVICES REVIEW IMPLEMENTATION BULLETIN 1 APRIL 2000
INTRODUCTION
The Acute Services Group is aware that many of those working in the NHS in Scotland are interested in having information about the implementation of the programme set out in the report of the Acute Services Review, which was published in June 1998.

MANAGED CLINICAL NETWORKS
The concept of Managed Clinical Networks has been given priority by the Acute Services Group. The Group's first task was to draw up the core principles which should underpin all Managed Clinical Network development, and these were set out in MEL(1999)10. MEL(1999)53 invited bids for demonstration projects, either related to a regional vascular Managed Clinical Network or a Managed Clinical Network in neurology with particular reference to stroke. A vascular demonstration project is already under development in Lanarkshire, and the neurology MCN demonstration project involves a collaborative effort between Law Hospital and the Institute of Neurological Sciences at the Southern General Hospital, Glasgow. It is anticipated that further vascular networks will be proposed to the Acute Services Group as the year unfolds.
A national seminar on Managed Clinical Networks was held in Dundee in October 1999 and a written report will shortly be circulated to participants. Copies will be available from the Secretariat to the Acute Services Group and the text will also be available on the Internet. Contact details are given at the end of the Bulletin.
CORONARY HEART DISEASE TASK FORCE
The overall aim of the Task Force is to develop a clinical network of cardiac services in Scotland. In order to achieve this objective, the Task Force has been assigned the following remit:

The Task Force found that there was additional capacity in the 4 existing cardiac surgery centres which would allow them to undertake almost another 500 CABGs a year. Health Boards have therefore been encouraged to move to a target of 600 CABGs per million population by 31 March 2000, in line with the target set by the British Cardiac Society.
DATABASE SUB-GROUP
Remit: To develop a national database based on routine patient management data in primary care, drawing on existing datasets and work currently in progress on CHD data collection as appropriate.

The sub-group has concluded that more information is needed about the incidence and prevalence of CHD in the community, and that the database should be founded in primary care, with electronic links to the range of existing CHD databases, including those held by ISD. Primary care data would be collected routinely, based on a minimum dataset. This dataset has been designed to provide demonstrable benefits to GPs in terms of patient management and advantage is being taken of the development of the new GPASS system to take the database proposals forward.

The Sub Group has devised a standard form of categorisation of patients on the waiting list for cardiac surgery in Scotland, based on their clinical condition, and is currently finalising a standard referral letter.
ACCREDITATION SUB-GROUP
Remit:
The Sub Groups are being kept closely informed of progress in relation to the development of the template so that their work conforms to the emerging approach for all CSBS projects. At the same time they are being encouraged to contribute to this process generally and specifically through the experience gained from the pilots. The Sub Group is serviced by CSBS staff.

It is also the Acute Services Group's intention to give further consideration to the overall pattern of paediatric tertiary services in Scotland. It will also consider the recommendations stemming from the work of Scottish Colleges' Committee on Children's Surgical Services, which is looking at low volume, highly complex paediatric surgical procedures. The Acute Services Group is also considering the desirability of issuing guidance on children's surgical services.

AMBULATORY CARE CENTRES
The recommendation in the report of the Acute Services Review that there should be a major demonstration pilot of the concept has been given added impetus by the inclusion of the Scottish Executive's commitment in its Programme for Government for the launch in 2002 of a new generation of Walk-in/Walk-out Hospitals. A major conference on the subject was held last November and a Working Group of the Acute Services Group is being set up to produce general guidance. This will complement the range of local initiatives being undertaken in areas such as Greater Glasgow, Grampian, Forth Valley and Fife. The Group will wish to explore the development of concepts such as the ambulatory care nurse and the generic therapist.
CLINICAL STANDARDS BOARD FOR SCOTLAND
The creation of the Clinical Standards Board for Scotland as a Special Health Board from 1 April 1999 fulfilled the recommendation in Chapter 8 of the report of the Acute Services Review for the introduction of what the report called a National Standards Group to take responsibility for the process of reviewing and accrediting the services provided by the NHS in Scotland. The Board's work is well in hand, and it is concentrating on standards of services provided in relation to the 3 clinical priorities of the NHS in Scotland.
REMOTE AND RURAL AREAS RESOURCE CENTRE
Health Board Chairmen and others in the NHS with responsibility for serving rural communities have, since publication of the report of the Acute Services Review, developed proposals for the Remote and Rural Areas Resource Centre, which will be based in Inverness. Its overall aim is to sustain and develop health services for all of Scotland's remote and rural communities, and it is underpinned by £2m a year. Particular issues receiving attention will include:-
